The Compassionate Fund
Evergreen End of Life Care Foundation offers funds to support services such as End of Life Doulas, Music Therapy, Massage Therapy, and other needs that are not typically covered by insurance. Payments are made directly to the Doula/Therapist on a monthly basis. As Evergreen continues to grow, we will expand our offerings and increase the number of hours we are able provide. If you or a family member need help finding a Doula or Therapist please reach out to us. We have resources available to help you.
Application Process for Massage Therapy
The Massage Therapist submits a funding application on behalf of the client by completing the form below.
Evergreen will review the application and either approve it, deny it, or request additional information from the Therapist.
The pay rate starts at $60 per hour, depending on the length and complexity of the massage.
Renewal is contingent upon the availability of funds and the timely submission of monthly progress reports.
If the application is approved:
The Massage Therapist must submit the following:
Proof of Michigan Massage Therapy License
Proof of Liability Insurance
If the application is denied, we will provide the Therapist with a reason for the denial. Due to limited funds, we may not be able to approve all applications.
