Evergreen End of Life Care Foundation offers funds to support services such as End of Life Doulas, Estate Lawyers, Massage Therapy, and other needs that are not typically covered by insurance. Payments are made directly to the Doula/Therapist/Lawyer on a monthly basis. As Evergreen continues to grow, we will expand our offerings and increase the number of hours we are able provide. If you or a family member need help finding a Doula please reach out to us. We have resources available to help you.
Application Process for End of Life Doulas
The End of Life Doula applies for funds on behalf of the client by completing the application below.
Evergreen reviews the application and will either approve, deny, or request additional information from the Doula. The pay rate is $30 per hour and renewal is based upon available funds and monthly progress reports being submitted on time.
If the application is approved:
If the application is denied, we will provide the Doula with a reason for the denial. Due to limited funds, we may not be able to approve all applications.

** The background check must be completed at the doula’s expense using the ICHAT website link below. The cost is $10. Be sure to save your report before exiting the website. If you prefer to use a different background check service, please email us to discuss.

- Proof of insurance
- Proof of training and/or NEDA proficiency*
- Completed W-9 form
- Background check clearance**
