Evergreen End of Life Care Foundation offers funds to support services such as End of Life Doulas, Music Therapy, Massage Therapy, and other needs that are not typically covered by insurance. Payments are made directly to the Doula/Therapist on a monthly basis. As Evergreen continues to grow, we will expand our offerings and increase the number of hours we are able provide. If you or a family member need help finding a Doula or Therapist, please reach out to us. We have resources available to help you.
Application Process
The End of Life Doula or Therapist applies for funds on behalf of the client by completing the application below.
Evergreen reviews the application and will either approve, deny, or request additional information from the Doula/Therapist.
If the application is approved:
If the application is denied, we will provide the Doula/Therapist with a reason for the denial. Due to limited funds, we may not be able to approve all applications.

If the application is approved:
- The End of Life Doula (aka Death Doula) must submit the following:
- Proof of insurance
- Proof of training and/or NEDA proficiency
- Completed W-9 form
- Background check clearance
- The Music/Massage Therapist must submit the following:
- Proof of credentials
- Proof of insurance
- Completed W-9 form
If the application is denied, we will provide the Doula/Therapist with a reason for the denial. Due to limited funds, we may not be able to approve all applications.
